ARH Pharmacy Residency Application Process

The Application Process

ARH’s Postgraduate Year 1 (PGY1) pharmacy residency program at Hazard ARH Regional Medical Center is a twelve-month post PharmD training program.

Residency Candidate MUST provide/submit the following:

Curriculum Vitae
College Transcripts
Three Letters of Recommendation

2. Complete Individualized Assessment for Residency Training.

3. Complete ARH Online Application. (Click the link to complete)

Application Evaluation Procedure:

Complete applications will be reviewed by a residency selection committee of the Residency Program Director (RPD), Director of Pharmacy (DOP), Residency Program Coordinator, preceptors and current residents.

Application scoring criteria:

Hospital and pharmacy experience
Academic performance
Strength of letters of recommendation
Written communication skills
Extracurricular activities
Leadership roles
Perceived ability to achieve residency goals and objectives.

The RPD may contact candidates as needed for application clarifications. Averaged scores will determine candidate eligibility for interview. The RPD will contact candidates as necessary to schedule interviews or notify candidates of ineligibility for interview.

Interviews

Interviews may be conducted either virtually or on-site. Current COVID-19 incidence may necessitate exclusive use of virtual interviews. Interviews will be attended by representatives of the selection committee. A combination of individual and group scheduling may be utilized for on-site interviews.

Rank and Match Procedure

Once all interviews are complete, the residency selection committee will rank candidates. It is the policy of ARH to provide equal employment opportunities without regard to race, color, religion, national origin, gender, age or handicap. In accordance with the rules of the National Matching Service, the Hazard ARH Regional Medical Center PGY1 Pharmacy Residency Program agrees that no person at this site will solicit, accept or use any ranking-related information from any residency applicant.

Phase II of the Match

The application evaluation and interview process will be exactly as Phase I of the Match but on an expedited time frame.

The following questions allow the residency applicant to conduct a self-assessment and provide information that will be utilized to help determine if the applicant is a good fit for the Hazard ARH Regional Medical Center’s PGY1 Pharmacy Residency Program. The answers will be evaluated by a residency candidate evaluation committee comprised of the Residency Program Director, Residency Program Coordinator, Director of Pharmacy, select preceptors and current residents. Please answer the questions in a narrative format.

1. State your career goals both short term (three to five years) and long range (ten years).(Required)

2. Describe your current practice interests.(Required)

3. What do you consider your strengths, and how do you plan to continue to build upon these strengths? This should include your professional and personal strengths.(Required)

4. What do you consider your weaknesses, and how would you like to improve these through your residency training?(Required)

5. Given your listed career goals, interests, strengths, and weaknesses, list at least three (3) goals that you wish to accomplish during your residency.(Required)

6. What areas of training would you like to concentrate on during your residency program? (List in order of importance)(Required)

7. Describe the frequency and type of preceptor interaction you feel to be ideal. Where do you see the preceptor fitting into your professional development and maturity?(Required)

8. Describe the following situations briefly:

a. A time when your preceptor or team rejected your recommendation. Include how you responded.(Required)

b. A time when you had to be an advocate for a patient or yourself.(Required)

c. A situation where you had to use supervisory or leadership skills during your pharmacy education.(Required)

d. A time when you became involved in a professional or social cause.(Required)

9. Describe how you would counsel a newly diagnosed Afib patient who is afraid to take a novel anticoagulant.(Required)
