- Copy of License (if applicable)
- Copy of Drug Enforcement Registration (if applicable)
- Copy of Educational Commission for Foreign Medical Graduate (ECFMG) Certificate (if applicable)
- MSPE and/or three references from supervisors, educators (two of which must be from these categories) or peers (if applicable, transferring resident
Must also meet eligibility requirements to obtain and maintain a training license in the State of Kentucky which requires demonstration of passing scores on one of the following sets of examinations:
- USMLE Steps 1 and 2 Clinical Knowledge (CK), Clinical Skills (CS)
- COMLEX Level 1 and 2 Cognitive Evaluation (CE), Performance Evaluation (PE)
- Medical Council of Canada Qualifying Examination (MCCQE) Parts I and II
- SPEX and PLAS (under circumstances that would mandate verification of competency, i.e., extended absence from clinical medicine)
Foreign medical graduates must supply proof of an Educational Commission for Foreign Medical Graduates (ECFMG) certification. Non-citizens must be a Lawful Permanent Resident, J-1 Visa holder, or have other valid work authorization.
